- Registered Nurse/Clinical Supervisor: Coordinates and supervises home nursing care from the agency, based on agency P & P, through the competent application of the nursing process. Manages field staff. Responsible to the Director of Nursing.
- Registered Nurse/Field Nurse: Provides and directs the provision of home nursing care, based on agency policies and procedures, through the competent application of the nursing process. Case manages a group of home care patients by overseeing care and other care givers. Responsible to the RN Case Manager and Director of Nursing.
- Licensed Practical Nurse/Field Nurse: Performs selected nursing care under the direction of the registered nurse and general supervision of the director of nursing. Supervision conducted every six months. Responsible to the RN Field Nurse, RN Case Manager and Director of Nursing.
- Home Health Aide: Provides support personal care services, under the supervision of the appropriate professional staff that assist the client or family in the achievement of physical and emotional comfort. Responsible to the RN Field Nurse and RN Case Manager.
- Physical Therapist: Administers to clients, according to physician’s orders, various evaluations and therapeutic procedures and maintains agency records on clients. Responsible to the Director of Nursing and RN Case Manager. Directly supervises and evaluates Physical Therapist Assistants as assigned.
- Physical Therapist Assistant: Carries out treatment, implementation and documentation of client status for home health clients under the direction of the registered physical therapist (RPT). Responsible to the Therapy Coordinator and Physical Therapist.
- Occupational Therapist: Responsible for the evaluation, planning, implementation, treatment and documentation of client status for home health clients. Responsible to the Director of Nursing, RN Case Manager and Therapy Coordinator.
- Speech Therapist: Responsible for assessments and treatments of speech and language disorders, including development, mental, and organic involvement. Responsible to the Director of Nursing, RN Case Manager and Therapy Coordinator.
- Medical Social Worker: Assists and counsels clients and families with financial, social and emotional concerns. Responsible to the RN Case Manager and Director of Nursing.
